Access to Records

In accordance with the Data Protection Act 1998 and the Access to Health Records Act, patients have the right to request access to their medical records. To do so, a formal request must be made through the Practice by completing a designated form, which requires specific details about the records being sought. An administration fee may apply to cover the cost of processing the request. It is important to note that no information will be disclosed without the explicit consent of the patient, unless there is a legal obligation to release it. Therefore, the completion of the form is essential to initiate and process any request for access.
