Rachael is the mental health practitioner who works alongside our doctors at the surgery. She is able to offer mental health support for adult patients.
She can:
- Provide a holistic assessment to help identify patient needs
- Support patients with brief Interventions
- Identify a need for onward referral
- Help identify and signpost to Community groups and/or events

If you are 25 years of age and older and would like support for drugs and alcohol then please contact Change Grow Live Service – click on this link and complete a SELF-REFERRAL.
If you are below 25 years of age and would like support for drugs and alcohol then please contact the No Limits Service – click on this link and complete a SELF-REFERRAL.
Both these services are a FREE NHS support service who can offer you support through your journey to recovery.
If you are an ADULT and you are concerned about your hearing, you can ask for a hearing test with our local service by self-referring.

Testicular Lump Service
This is a SELF-REFERRAL service for:
-Male patients aged 18-50 years of age
–WITH a painless, testicular lump.
You can self-refer by calling the Rapid Investigation Service (RIS) on 0300 123 1385 (Monday to Friday 9am-4pm but a voicemail service is available outside of these hours). This is currently only a telephone self-referral service.
You must watch the video by clicking on this link to complete a self-examination of the testicles before calling.
Patients should receive an appointment within 2 weeks for an Ultrasound Scan and have an appointment the same day with a specialist. The hospital radiology team will inform the patient of this appointment via text, email, or letter.
PLEASE do NOT use this service if:
-You have been seen within the last 12 months by a hospital clinician for the same testicular concern
-You have pain in the testicle(s)
-You are a temporary residents/students or you are planning to move within 28-days of your self-referral.

If you are concerned about your child’s hearing, you can ask for a hearing test with our local service by self-referring. You can do this by completing the “children’s hearing form – self-referral” which you can find below.

Children’s hearing form – self-referral
In addition to minor illnesses, you can now self-refer to a pharmacy for 7 conditions without needing to contact us at the surgery. The pharmacy can now asses and prescribe for you for 7 conditions. Please note if you do contact us at the surgery for these 7 conditions the clinicians may redirect you to the Pharmacy First service if they feel you maybe suitable for this service.

If you think you have OTITIS MEDIA (EARACHE) then please attend your local pharmacy for the PHARMACY FIRST SCHEME. They will be able to assess and prescribe for you if you are:
-Between 1 – 17 years of age
-Earache
-Holding, tugging or rubbing the ear
-Discharge from the ear
-Younger children; temperature, crying, poor feeding, restlessness, behavioural changes, cough, runny nose

If you think you have SINUSITIS then please attend your local pharmacy for the PHARMACY FIRST SCHEME. They will be able to assess and prescribe for you if you are:
–12 years of age and over
-Nasal blockage (obstruction or congestion)
-Nasal discharge
-Facial pain/pressure (or headache)
-Temperature
-Green/yellow discharge from the nose
-Reduction /loss of the sense of smell
-Cough
-Toothache

If you think you have a SORE THROAT then please attend your local pharmacy for the PHARMACY FIRST SCHEME. They will be able to assess and prescribe for you if you are:
–5 years of age and over
-Painful throat, especially when swallowing
-Dry, scratchy throat
-Redness/pus in the back of the mouth
-Temperature
-Swollen neck glands
