Live within boundary area?

To be eligible to register at the practice, you must live within the practice catchment area.

Catchment Area

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photo ID.

You MUST complete a registration form (GMS1) AND a health questionnaire to complete registration – if you only complete a registration form your registration will not be completed –  If your address is not in the practice area the forms will also not be processed so please phone to check registration

Please ensure you have at least 4 weeks supply of your medication from your previous GP when you register.

If you are on repeat medication you will need to see a doctor first to authorise your inclusion on to our repeat prescription system.

Medical treatment is available from the date of registration. Please contact reception for further information.
