To be eligible to register at the practice, you must live within the practice catchment area. Please check if you are in our catchment.

Complete the online new patient registration form (GSM1+Health Q’s) using the link below. Please ensure you fill in ALL questions as missing information will delay your registration with us.
If you are new to the UK, you MUST put the date you arrived in the UK.

New registrations can take up to 1 week to be processed and you will receive a text message once completed.
If you are unable to complete a new patient registration form online then please speak to the reception team at the surgery when we are open to collect a paper registration form.
We may require proof of address and evidence of identity to provide certain services.

Please ensure you have at least 4 weeks supply of your repeat medication from your previous GP when you register.
